If you have questions about which form you need or how to complete a form, please contact your Membership Department. Completed forms should also be directed to the Membership Department.

Changes and Updates to Your Information:

  1. Change of Address Form
    Use this form to keep your mailing address up-to-date with ACTRA so you will receive cheques or other important mail.

  2. Agent Authorization Form
    Update ACTRA and give your permission for cheques to be forwarded to your current agent with this form.

  3. Name Change Form
    If you change your legal name or your professional name, update ACTRA using this form.

  4. Branch Transfer Form
    If you move to another location and will be working under a different branch of ACTRA you will need to complete and provide this form.

Permit Applications:

  1. Commercial (NCA) Permit Application
    Complete this form to apply for a permit to work on a commercial under ACTRA's jurisdiction.

  2. IPA Permit Application
    Complete this form to apply for a permit to work on an independent or broadcast film, television or new media project under ACTRA's jurisdiction.
